Abstract
A chronic rupture of the patella tendon is uncommon injuries. Because they accompanied by atrophy and contracture of quadriceps muscle, great deal of scar formation and poor condition of remaining patella tendon, these ruptures are often difficult, technically, to reconstruct to reestablish a normal functional extensor mechanism of the knee. We report a case of repair of chronic patella tendon rupture with an Achilles tendon allograft, which performed 50 years later after the injury. Thirty months postoperatively, the patient attained full active extension and 120 degrees of flexion and gained 70% of quadriceps strength. The technique accomplished the preoperative goal of restoring quadriceps strength, restoring the anatomic patella position and alignment and improvement of functional gait. The patellar tendon reconstruction with an Achilles tendon allograft, provided an acceptable result in 50-year-old patella tendon rupture.
